Health Coordination & Navigation

Navigating patients and carers through the health system including NDIS clients.

Acting as the central point of contact between services.

Logistical support such as scheduling appointments, assessments, discharges.

Liaising with primary care team to relay areas of concerns.
Support & Advice

Support clients acknowledging them as the expert at the center of their health experience.

Virtual health consults to follow up for support, to address issues and monitor progress towards health goals.
