  • Explore and refer to alternative health options including acupuncture, Chinese medicine and plant medicine practitioners.

  • Acting as the central point of contact between services.

  • Logistical support such as scheduling appointments, assessments, discharges.

  • Liaising with primary care team to relay areas of concerns.

  • Support clients acknowledging them as the expert at the center of their health experience. 

  • Virtual health consults to follow up for support, to address issues and monitor progress towards health goals.

  • Answer patient and family questions and source additional information for them.

  • Emotional support and advice to client and carer.
